Division of Health & Vital Statistics

The Division of Health and Vital Statistics is responsible for recording, maintaining, and making available all records of birth, marriage and death. In addition, the Division of Health and Vital Statistics performs all appropriate functions associated with:

  • Processing applications for City dog licenses
  • Health Ordinance and Regulations Compliance Review
  • Processing applications for food establishments
  • Maintaining City Board of Health records

Registrar of Vital statistics

Vital records are kept of all births, deaths and marriages occurring within the City of Burlington. Contact the Registrar to obtain certified copies of:

  • Birth Records
  • Death Certificates
  • Marriage Certificates

$25.00 first copy
$2.00 each additional copy

Marriage Licenses

Couples must apply for a marriage license in the place where the bride resides. If the bride’s residency is out of state, you must apply in the town where the wedding ceremony will occur. If you plan to get married in the City of Burlington, please contact the Registrar, and provide the following:

  • Proof of residency
  • Social Security numbers
  • A witness age 18 or over
  • Maiden names of both Mothers
  • Name and address of person performing the wedding ceremony
  • Address where the wedding will be held
  • Divorce papers from previous marriages, if applicable
  • Death Certificate from previous marriages, if applicable

Applications are available online or in person.

Marriage License Application

Marriage License Application Fee: $28.00.

Please note that there is a 72-hour waiting period from date of application to date of issuance, excluding the day of the wedding.

Do not sign the marriage application form before going to the Local Registrar. You must sign the application, under oath, in the presence of the issuing authority.

Food Establishment License Renewal Application

Dog Licenses

All dogs over the age of 6 months must be licensed annually. Dog licenses should be renewed each year by January 31st of each year. There is a $15.00 late fee for all registrations after January 31 st .

Please have:
  • certificate of rabies vaccination
  • proof of spaying or neutering

Dog License Fees:
Dogs spayed or neutered: $8.00/yr.
Dogs NOT spayed or neutered: $11.00/yr.

Just a friendly reminder to make sure your dogs are properly licensed, on a leash when outside your property and that they are not causing damage or disturbing the peace. To clarify any misinterpretations, we have repeated the Animals ordinance below, including possible penalties.

Chapter 84. Animals, Article II. Dog Licensing
84-4. Regulations [Amended by Ord. No. 6-2006]:
  1. Registration tags. A metal registration tag shall be issued for each dog licensed. Each dog licensed shall wear a collar or harness with its registration tag securely fastened to it.
  2. Improper use of registration tags. No person, except an officer of the City in the performance of his/her duties, shall remove a registration tag from the collar of any dog without the consent of the owner, nor shall any person attach a registration tag to a dog for which it was not issued.
  3. Running at large. No owner of any dog shall permit it to run at large upon any street, park, public building or any other public place within the City.
  4. Leashing of dogs. No owner of any dog shall permit it to be upon the public streets or in any of the public places of the City unless such dog is accompanied by a person over the age of 12 years and is securely confined and controlled by an adequate leash not more than six feet long.
  5. Property damage; responsibility of dog owner.
    • No owner of any dog shall permit it to do any damage to any lawn, garden, shrubbery, flowers, grounds or other property.
    • In addition to any fine levied as a result of violations of this article, the owner of any animal which causes damage to another's property shall be responsible for the damage caused.
  6. Disturbing the peace. No person shall keep, harbor or maintain any dog which habitually barks or cries between the hours of 10:00 p.m. and 6:00 a.m.
  7. Confining of female dogs. Every female dog in season shall be kept confined in a building or secure enclosure or in a veterinary hospital or boarding kennel, in such a manner that such female dog cannot come in contact with another dog, except for intentional breeding purposes. Nothing herein shall prevent the owner from walking and exercising such dog on that owner's property, provided that the dog is controlled and secured by an adequate leash. The Animal Control Officer shall take into custody and impound any female dog in season off the premises of the owner or of the person keeping or harboring such dog.

The Division of Animal Control does not issue dog licenses. All dogs living in the City of Burlington over the age of 6 months must be licensed annually. Cats need not be licensed. Dog licenses are available from the Health Division / Registrar of Vital Statistics.

Be prepared to provide:
  • Certificate of rabies vaccination.

Dog licenses should be renewed each calendar year during the month of January! After that, registrations will incur a late fee of $15.00. The Division of Animal Control does not issue dog licenses, and licenses are no longer offered at Keegan Center.
